The role you’ll contribute:
Clinical Transplant Coordinators are members of a multidisciplinary team who work together to provide individualized care to patients who are seeking and/or have received an organ transplant. Responsibilities include coordinating patient care through all phases of transplantation which include pre-operative, peri-operative, and postoperative care and follow up. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Participates in 24-hr on-call schedule
Facilitates/coordinates individualized evaluations for potential organ recipients or living donors which includes: collection of patient medical data, patient/family education, identification of co-morbidities/contraindications to transplant (or living donation), collaboration with transplant team members.
Coordinates patient care in outpatient setting to achieve optimal healthcare recovery. * Reinforces patient education * Monitors/assesses patient for potential post-transplant complications.
Coordinates the entry and the integration of patients into the program by facilitating appointments for tests, procedures, and required family services. Coordinates and expedites all BMT protocols and informed consents. Communicates and collaborates with multi-disciplinary team and other healthcare personnel relevant to the program and the status of upcoming patients
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
Bachelor's degree
1-year of transplant, critical care
1-year of leadership experience
Critical care experience or palliative care of patients with life-limiting or life-threatening illness.
RN - Registered Nurse - State Licensure and/or Compact State Licensure State of Florida or multi state.
BLS - Basic Life Support
Preferred qualifications:
BSN or Bachelor's degree in health related field
Teaching/preceptor experience
Transplant care experience
Nursing case management
