This position is located in the Medical Service, at the Boise VA Medical Center, Boise, ID. This position provides support to the Cardio-Vascular Lab. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: You must be proficient in spoken and written English per 38 U.S.C. ? 7402(d) Education: There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. Licensure or Certification: Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-09 Experience: At least 1 year experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of echocardiography services. Demonstrated Knowledge, Skills, and Abilities: In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to Echocardiography. Knowledge of JCAHO and other regulatory requirements. Ability to communicate effectively and professionally with employees at varying grade levels. Certification: Registration by Cardiovascular Credentialing International (CCI) as a Registered Cardiac Sonographer (RCS) or by the American Registry of Diagnostic Medical Sonographers (ARDMS) as a Registered Diagnostic Cardiac Sonographer (RDCS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable. Preferred Experience: Demonstrates advanced knowledge of complex and nonstandard echocardiography treatment and examination procedures and techniques. Performs in hospital setting all ultrasound examinations ranging from the routine M-Mode, 2Dimensional, Spectral Doppler and Color Doppler to the highly specialized exams, including Transesophageal and Stress Echocardiograms. Perform extensive physical examination of the on patients, including but not limited to bilateral blood pressures, listening for bruits, and taking a major history. Performing routine Carotid duplex, routine Upper and lower extremity arterial duplex studies, routine upper and lower extremity venous studies and renal doppler exams. Advanced skill sets for vascular exams include Subclavian steal testing, vein mapping for AV fistula creation, graft assessment of Gortex grafts, reverse saphenous vein grafts, jump-interposition grafts, femoral to femoral vein bypass grafts and axillary to femoral bypass grafts. Physical Requirements: Work is primarily active; position requires lifting up to 100 pounds; moving; operating and handling equipment; assisting patient to and from the bed; walking and prolonged period of sitting. ["Duties include but are not limited to the following: Review and triage Echo and Vascular consults, assesses for appropriate order and diagnosis, and recommends appropriate scheduling of exams Obtains a patient history and perform appropriate physical assessment based on patient presentation and type of exam to include observation, inspection, auscultation, and palpation. Identify and record anomalies indicative of disease and injury or other medically significant conditions from ultrasound imaging including M-Mode, 2D, Spectral and color doppler. Controls technical factors on ultrasound units which will affect time, gain, amplitude, velocity, and frequency attenuation, absorption, reflection scattering and refraction and diffraction. Operates the instrument to extract the highest quality image utilizing the appropriate transducer positioning and frequency, Doppler angle, focal zones, gain, depth, color map, wall filter, and sample volume. Performs standardized vascular ultrasound procedures and/or examinations of upper and lower extremity and carotid exams. Performs standard cardiac ultrasound procedures and/or examinations of cardiac transthoracic, saline and echo-enhancing contrast echocardiography, stress echocardiography, and transesophageal echocardiography. Identify when the severity of the disease is indicative of further testing and requesting permission to proceed with other testing thus enabling for more thorough results for the patient. Provides objective data to document progression or regression of disease. Prepares preliminary technical findings of exam using structured reporting tool. Recognizes adverse reaction or critical findings and contacts appropriate when necessary. Collaborates with the medical staff to establish comprehensive integrated treatment plans which identify appropriate examination procedures and techniques, advises the referring physician with a preliminary result of the exam. Audits exam PACS and notifies interpreting physicians of any outstanding, uninterpreted exams. Ensure all consults are completed. Finalize encounters with appropriate interpretation, diagnosis and code based on facility guidelines.
