The computed tomography/cardiovascular (CV) technologist has the responsibility for the coordination and performance of the computed tomography and angiography areas. He/she works closely with the technologists, radiologists, clinicians, nurses and clerical staff to assure optimum quality control, efficient coordination of activities, high quality diagnostic/therapeutic exam results, and patient satisfaction. All applicants must be certified in general radiologic technology by the American Registry of Radiologic Technology, Radiography (ARRT) (R). Advanced ARRT certification is required for assignments that include computed tomography (CT), magnetic resonance imaging (MRI), or Mammography (M) duties performed independently, as applicable. Advanced certification indicates that the incumbent can operate independently and has demonstrated specific clinical competency in the appropriate specialty and taken and passed the designated examination. Grade Determinations: (1) Knowledge of Current Radiologic Technology Practice. To be creditable, experience must have demonstrated possession of the knowledge, skills, abilities, and other characteristics (also referred to as clinical competencies) associated with current radiologic technology practice. This may have been evidenced by the equivalent of one year of active practice, which is paid or non-paid employment as a DRT as defined by ARRT. (2) Quality of Experience. Experience is only creditable if it was earned after completion of the basic certification requirements identified in paragraph 2b above. Experience as a graduate DRT is creditable provided the candidate functioned as a DRT and subsequently passed the certification examination. (3) Part-Time Experience. Part-time experience as a DRT is creditable according to its relationship to the full-time workweek. For example, a DRT would receive one week of full- time credit for each two weeks of half-time work. Specialized Assignments. Specialized areas of radiologic technology include general diagnostic radiologic technology (R), bone densitometry (BD), mammography (M), diagnostic ultrasound (S), computed tomography (CT), magnetic resonance imaging (MR), interventional radiologic technology (IR), quality management (QM), picture archival communication system (PACS) technologist, MRI safety officer (MRSO), imaging administrator, clinical program director, and clinical instructor. Computed Tomography (CT). This specialty modality requires specific knowledge of cross sectional human anatomy and its application in spiral, and/or, multi-slice computer tomography, inclusive of 3-D reconstruction scans, including drainages, biopsies, and peripheral vascular examinations. The technologist requires specific knowledge and training in the location, appearance, and function of the various major and minor systems susceptible to radiological illumination; to interpret the examination request accurately; to understand the functioning and inter-relationship of the various organs; to use the methods and techniques which will identify organs appearing on the digital display monitor, or on film, and the various stages of the examination to judge the acceptability of the image and/or scan for diagnostic use and to emphasize the aspects of interest to the physician. DRT assignments that include performance of independent duties in this subspecialty require advanced ARRT certification. Interventional Radiography (IR). Technologists working in this special assignment may function as vascular interventional (VI), cardiac interventional (CI) and/or cardiovascular-interventional (CV) technologists which use specialized equipment to perform diagnostic angiographic procedures and complex vascular and nonvascular interventional and therapeutic procedures. This specialty requires additional knowledge of vascular systems and major vessel anatomy. The technologist must be knowledgeable in the specialized equipment employed for digital subtraction systems and interventional procedures. The technologist administers contrast media under the supervision of the staff radiologist and confers with the radiologist to establish requirements regarding contrast agents, vital signs, medications, and physiologic monitoring to perform procedures. IR technologist must be knowledgeable in digital imagining and PACS. Diagnostic Radiologic Technologist, GS-9 (CT or IR) (1) Experience. At least one year of experience equivalent to the next lower grade level (GS-8), directly related to the position being filled that demonstrates the clinical competencies described at that level. (2) Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. DRTs at this grade level may be appointed to one of the following assignments: (a) Diagnostic Radiologic Technologist. Employees at this level are fully functional as an advanced DRT and carry out their assigned tasks independently. DRTs at this level may have varying assignments including special and complex imaging procedures beyond the full performance level, clinical instruction, and basic QM type duties within the program. Regardless of the nature of the specific assignment, the work must be of sufficient scope and complexity to meet the knowledge, skills, and abilities to perform at this level. This is done to minimize hazards associated with invasive procedures and involves the use of sterile packs, gloves, syringes, guide wires, catheters, clean-up trays, etc., after exam is completed. Adheres to hospital infection control and safety policies/techniques with emphasis on sterile technique; the storage of sterile supplies; the disposal of body fluids and 'sharps'; proper attire; hand washing practices. G. Works with the radiologist as a member of the radiographic team. Must assist and anticipate the needs of the radiologists to ensure positive outcomes. Must be continuously on the alert for the patient's condition, sterile field, catheter and guide wire sizes and configurations, etc. The alertness of the individual is essential to prevent contamination, recognize patient reactions, and anticipate the physician's needs. H. Prepares contrast materials. These contrast materials are used in the exams performed in the special procedure area and the technologist must know the effects and reactions to the contrast. Improper use or administration of the contrast materials run the risk of patient mortality and, therefore, it is essential that a thorough understanding of the contrast material use and delivery methods are possessed by those who prepare it. In addition, must be continuously alert to the patient's condition and other disorders. Reactions on the part of the patient require immediate attention so that harm does not come to the patient or equipment. Calculates eGFR values and utilizes iStat analyzer. I. Practices radiation safety in order to reduce exposure to patients, staff and self. This is achieved by the use of lead aprons, lead gloves, lead shields, lead lined walls and collimation. Film badges are worn by the technicians in compliance with radiation regulations. The incumbent must be aware of and practice the safety policies/procedures of the facility, and participate in quality assurance activities as directed. J. Performs image reconstructions on advanced workstations as directed by the radiologist or as part of a given study. K. Maintains records and files of exams performed on patients. This includes the maintenance of patient logbooks, registration and editing of patient exams in the hospital information system and radiology information system, copying of images (to CD or film), and ensuring procedures are in the HIS/RIS and available for review. L. Performs irregular hour emergency scans on standby, 24 hours per day, coverage for both IR and CT. Requires the technologist be available within 45 minutes from receipt of call so that proper and effective emergency care can be given to the patient.
