The VA Central Iowa Health Care System in Des Moines, IA is currently recruiting for two(2) full-time Licensed Practical Nurse to work on the Community Living Center CLC-2 Unit with the Extended Care and Rehabilitation Service Line. The LPN is responsible for providing care to a veteran population with widely diverse medical, skilled, mental health and end-of-life needs. The incumbent is responsible for direct patient care activities under the supervision of the Registered Nurse. The LPN is responsible for goals and interventions that promote veterans in attaining maximum functional potential. This is accomplished through but not limited to modalities such as ambulation, bowel and bladder training, dressing, grooming, bathing, oral hygiene, range of motion exercises and fall prevention modalities. Duties include, but are not limited to the following: Perform duties of a Licensed Practical Nurse at a competent level Implements orders and provides care to a variable resident population Performs duties/assignments with supplemental verbal instructions that are given by a Registered Nurse Observes, identifies, and responds to resident's need for care Observes for physical and/or emotional changes in resident's condition promptly and accurately notating deviations from normal to the RN and/or Physician Responsible for supporting factors that impact customer satisfaction Considers all characteristics of the individual including age, life stage, state of health, race and culture, values, and previous experiences to provide optimum veteran centered care- Administers medications and procedures per established policies and guidelines Collaborates with interdisciplinary team members to positively influence veteran care outcomes Work with team members supporting veterans and family members utilizing a holistic approach through end-of-life Other duties as assigned within the LPN scope of practice Preferred Experience: Flexibility in schedule and assignments for a 24/7 facility Highly motivated, self-directed individual with strong work ethics 1-2 years of LPN experience Poised and articulate in communicating with individuals of varied education and ethnic backgrounds Rehab/Restorative Care Experience Collaborative and effective team member Organizational and time-management skills Work Schedule: Full-time Nights, may include 8 hour and 12 hour shifts including weekends and holidays (11:30pm-8:00am, 7:30pm- 8:00am) Financial Disclosure Report: Not required
